HIV-1-related cardiovascular disease is associated with chronic inflammation, frequent pericardial effusions and increased myocardial oedema

چکیده
Background Treatment for HIV has been associated with a near-normal life expectancy, though with increased cardiac morbidity and mortality. Mechanisms of heart disease in patients infected with HIV remain unclear. Coronary artery disease and myocardial dysfunction may be partly related to untreated chronic inflammation. Cardiovascular magnetic resonance (CMR) imaging allows a comprehensive assessment of myocardial structure, function and tissue characterization. We investigated, using CMR, the presence of subclinical cardiac inflammation and myocardial disease in asymptomatic HIV-infected individuals.

HIV-1-Related Cardiovascular Disease Is Associated With Chronic Inflammation, Frequent Pericardial Effusions, and Probable Myocardial Edema.
BACKGROUND Patients with treated HIV infection have clear survival benefits although with increased cardiac morbidity and mortality. Mechanisms of heart disease may be partly related to untreated chronic inflammation. Cardiovascular magnetic resonance imaging allows a comprehensive assessment of myocardial structure, function, and tissue characterization. We investigated, using cardiovascular m...
متن کاملCardiovascular magnetic resonance characterisation of pericardial and myocardial involvement in patients with tuberculous pericardial constriction with and without HIV co-infection
Background Tuberculosis pericarditis (TBP) is the most common cause of a large pericardial effusion in the developing world, accounting for 70% of effusions in a case series from South Africa; and has a high mortality related to pericardial tamponade, constrictive pericarditis, arrhythmias and heart failure. Manifestations of TBP include pericarditis with pericardial effusion, effusive-constric...
متن کاملAdenosine deaminase activity--more than a diagnostic tool in tuberculous pericarditis.
AIM To improve the understanding of factors that influence adenosine deaminase ( ADA) activity in large pericardial effusions. METHODS A prospective study was carried out at Tygerberg Academic Hospital, South Africa. Patients underwent echocardiographically guided pericardiocentesis. ADA activity, as well as biochemistry, haematology, cytology, and in some cases, histology, were determined. H...
متن کاملHuman immunodeficiency virus‐associated heart failure in sub‐Saharan Africa: evolution in the epidemiology, pathophysiology, and clinical manifestations in the antiretroviral era
The survival of patients with human immunodeficiency virus (HIV)/acquired immunodeficiency syndrome (AIDS) who have access to highly active antiretroviral therapy (ART) has dramatically increased in recent times. This review focuses on HIV-associated heart failure in sub-Saharan Africa (SSA). In HIV infected persons, heart failure may be related to pathology of the pericardium, the myocardium, ...
متن کاملAbnormal Myocardial Function Is Related to Myocardial Steatosis and Diffuse Myocardial Fibrosis in HIV-Infected Adults.
BACKGROUND Impaired cardiac function persists in the era of effective human immunodeficiency virus (HIV) therapy, although the etiology is unclear. We used magnetic resonance imaging (MRI) to measure intramyocardial lipid levels and fibrosis as possible contributors to HIV-associated myocardial dysfunction. METHODS A cross-sectional study of 95 HIV-infected and 30 matched-healthy adults, with...
